- Evaluation of the Relative Efficacy and Safety of Prasugrel and Clopidogrel in Medically Managed High Risk UA/NSTEMI ACS Population

Background: The term "acute coronary syndrome" encompasses unstable angina and non-ST-segment elevation myocardial infarction (UA/NSTEMI) and ST-segment elevation myocardial infarction (STEMI). Antiplatelet therapy is one of the cornerstones of therapy in UA/NSTEMI.
Objective: To compare efficacy and safety of Prasugrel and clopidogrel both theinopyridines antiplatelet drugs in high risk (TIMI Score 3 or more) medically managed UA/NSTEMI.
Materials and Methods: A prospective, randomized study was conducted in GNDH, Amritsar. 100 patients were included, 50 patients received Prasugrel and 50 received clopidogrel. Outcomes like angina episodes, bleeding, stroke, ischemic ECG changes, and arrhythmia were compared during hospital stay and follow-up for 3 months.
Results: Prasugrel was associated with significant lower incidence of major adverse cardiac event (MACE) 9 compared to 19 with clopidogrel during hospital stay. During follow up for 3 months 2 events occurred with Prasugrel and 3 with clopidogrel which were non-significant.
Conclusion: Use of Prasugrel was associated with less number of MACE than the patients who were on clopidogrel. Although for the individual adverse coronary events, except for angina there was no statically significant difference, but when the total MACE observed during the study was compared, it was significantly less in the patient on Prasugrel therapy. Safety of the Prasugrel in present study was identical to clopidogrel.

Background: Several studies have been conducted to assess the stroke awareness among the general population. Few studies conducted on stroke patients have revealed inadequate knowledge regarding various aspects of stroke. No comparative study of stroke awareness among patients with stroke and patients of cardiovascular disorders who are at high risk. Material and Methods: A comparative cross-sectional survey was conducted on adult stroke survivors (n=201) and patients of cardiovascular disorders who are at high risk (n=201) in neurology and cardiology wards and outpatient clinics. Demographic profile, clinical profile and knowledge questionnaire were used for data collection. Consent was taken from the patients and ethical clearance was obtained.
Results: Both the groups were found to be statistically comparable with respect to age (p=0.280) and sex (p=0.411).Awareness regarding risk factors and warning signs of stroke was significantly higher (p=<0.05) in patients who are at high risk as compared to stroke survivors. More than half of the patients in both groups had <50% knowledge. Only 16% of the patients with stroke and 14% of the patients who are at high risk were aware that stroke could be due to cerebral hemorrhage. Only 12% of the patients with stroke and 11% of the patients who are at high risk knew that the stroke patients should be given immediate treatment within three hours.
Conclusion: Majority of the patients were not found to have adequate knowledge on risk factors, warning signs and need for immediate treatment. Hence, there is an intense need to educate the population at risk for stroke regarding risk factors and warning signs and immediate treatment.

This paper analyses the qualitative performance of microfinance services in Urban Punjab as perceived by the beneficiaries. The success of microfinance in achieving its social objectives, effectiveness of the systems, role of HR and the ultimate impact it has on its customers has been taken into consideration. For this, the customers were asked to fill the questionnaires and a comparison of formal and informal service providers was made. The factor analysis technique followed by weighted average score ranked the various performance indicators. Even though the loan policies of formal channels were found favorable, they could not transform the status of the poor.Keywords

An analysis of an unsteady free convective flow of a viscous, incompressible, electrically conducting fluid past an infinite hot vertical porous plate in slip flow regime has been carried out under the following assumptions: (i)the suction velocity normal to the plate is constant, (ii) the plate temperature is spanwise cosinusoidal fluctuating with time, (iii) the difference between the temperature of the plate and the free stream is moderately large causing free convection currents and (iv) the transversely applied magnetic field and magnetic Reynold number are really small and, hence, the induced magnetic field is negligible. The governing equations have been solved by adopting complex variable notations and the expressions for velocity and temperature fields obtained. The transient velocity, temperature, and amplitude the shear stress and heat transfer at the plate have been discussed.Keywords

The paper attempt to investigate the effect of injection/suction on the flow of electrically conducting fluid through a porous channel filled with porous material. A magnetic field of uniform strength is applied perpendicular to the plane of the porous channel. The effect of heat radiation, heat source and chemical reaction has been taken into consideration. The governing equations are solved analytically by using complex variable notation. The expressions for velocity, temperature, concentration, skin friction coefficient, rate of heat transfer and rate of mass transfer have been obtained. The effect of various parameters entering the governing equation have been evaluated numerically and expressed graphically and in tabular form.
