Chronic diseases are diseases of long duration and generally slow progression. Chronic diseases, such as heart disease, stroke, cancer, chronic respiratory diseases and diabetes are by far the leading cause of mortality in the world, representing 63% of all deaths (World Health Organization (WHO, 2010). Rapid improvements in health and longevity are dramatically changing the burden of illness throughout the world. In developed countries changes in lifestyle and improvements in the treatment of major causes of mortality have aged the population and increased the prevalence of chronic diseases. Poor countries that have achieved gains in life expectancy are also experiencing an increase in chronic disease. According to Centers for Disease Control and Prevention (CDC) more than 1.7 million Americans die of a chronic disease. Health promotion is integral part of nursing care. There are several way, by which individual can promote their health.Keywords

Aims and Objective:1] To assess the level of knowledge on self-monitoring of blood glucose (SMBG) among of diabetic patients. 2] Assess the Attitude on self-monitoring of blood glucose (SMBG) among of diabetic patients. 3] Find out the Correlation between level of Knowledge and Attitude on self-monitoring of blood glucose (SMBG) among diabetic patients. 4] Find out the association between levels of knowledge with selected demographic variables of diabetes mellitus patients. 5] Find out the association between attitudes with selected demographic variables of diabetes mellitus patients in Waghodia Taluka.
Methods: The interventional study was conducted andsample consisted of 200 patients of selected area of Waghodia Taluka. The tools used for data collection was a set demographic variables. Interview scale will be used to assess the knowledge and Likert scale will be used to assess the Attitude on SMBG. A planned teaching program on knowledge and attitude on SMBG among diabetic patients belongs to waghodia taluka was the intervention of the study. Content validity of the tool was ensured by verifying it with experts from the field of medicine and nursing. In the data gathering process, a pre-test was administered to assess the knowledge level of sample on the same day was conducted. After 7th day post test was administered using the same set of questionnaire to assess the knowledge.
Interpretation: The result showed that negative correlation between knowledge and attitude on SMBG in among diabetic patient belong to Waghodia Taluka.
Conclusion: The motivated the diabetic patient about knowledge and attitude on SMBG in diabetic patient. It has given a new avenue to the researcher to widen the horizon on move research aspect of knowledge and attitude on SMBG in diabetic patient.

Introduction: Children are like buds in a garden and should be carefully and lovingly nature, as they are the future of nation and citizen of tomorrow.(Jawaharlal Nehru). A child is the gift of god or greatest treasure of mankind. Childs health, wellbeing, safety and future are in the hands of parents from birth to death. The first year of child life is crucial laying the foundation to good health. At this time certain specific care and precautions are essential to ensure the survival of health of child to a future adult.
Objectives: To assess pre-existing knowledge regarding care of the neonate under phototherapy. To evaluate the effectiveness of the planned teaching program on care of the neonate under phototherapy. To find association between pre-test knowledge score regarding care of the neonate under phototherapy with demographic variables.
Materials and methods: The investigator used to pre-experimental research designed used (one group Pre-test Post-test design group). Nonprobability purposive sampling technique were used, sample size were 60 diploma internship nursing students at Gujarat state. The investigator used planned teaching program to assess the knowledge of diploma internship nursing students at Gujarat state. The conceptual framework for this study based on modified Health Belief Model from Becker M.H and Miaman L.A. Medicare Prepared. The data was analysis by using descriptive and inferential statistics. Paired t-test was used for evaluate effectiveness of planned teaching program.
Results: In the group of the students pre-test knowledge mean score was 15.18 (51.93%) and post-test mean score was 25.71(85.72%) and t-test value was 29.81 at the significance level 0.05 level df value was 59. So hypothesis H1 was accepted and hypothesis H2 was only associate with gender. Conclusion: Hence planned teaching program was affected for the diploma internship nursing students regarding care of the neonate under phototherapy.

Introduction: According to WHO (1994) Diabetes Mellitus is characterised by hyperglycaemia and disturbance of the carbohydrate, fat and protein metabolism that is associated with the absolute or relative deficiencies of insulin action or secretion. India has now been declared by WHO as the diabetes capital of the world. The choice of treatment and complaints is important for the metabolic control to be achieved by the diabetic patients. While diet, exercise and oral drugs are useful in controlling diabetes in many patients, it is now recognized that a large number of Type 2 diabetic patients require insulin therapy. The number of adults with diabetes in the world will rise from 135 million in 1995 to 300 million in the year 2025. The major part of this numerical increase will occur in developing countries.
Objectives: The objectives of the study were to determine the level of knowledge on prevention of complications of diabetic mellitus, evaluate the effectiveness of health education program in terms of gain in knowledge score among diabetes veterans, and find an association between level of pre test knowledge of diabetes veterans with their selected demographic variables.
Materials and methods: An evaluative research approach with pre-experimental one group pre-test and posttest design was used. 40 samples were collected by using convenient method from jalaram seva trust and community old age home care. Tool consisted of 6 demographical variables and 30 structured questioners was administered before and after HEP. The reliability of the tool was established by using test retest method. Data was analzed using inferential and descriptive statistics.
Result: Paired sample t-test revealed that the mean posttest knowledge score regarding prevention of complications of DM was significantly higher (24.23 ± 3.69) than that of mean pretest knowledge score (15.63 ± 3.91) (P< 0.05). Computed T value 12.908 is higher than table value t99 at P< 0.05. Hence hypothesis is accepted and thus shows that HEP was highly effective in increasing knowledge of prevention of complications of DM among Diabetes veterans.
Conclusion: The study finding revealed that health education programme was highly effective in improving knowledge of prevention of complications of DM in Diabetes veterans.

Background: Children are the most important asset and wealth of a nation. Healthy children make a healthy nation. The children under 15 years of age constitute about 40% of the population. The child is not a miniature, but an individual in his own right. The quality of childhood one has lived will determine the ultimate nature of the adulthood. Now a day, because of the rapid industrialization and urbanization, majority of young couples are employed, so unavoidably they get less time to look after their children. Under these circumstances, emotional, behavior and psychiatric problems are on the rise. Aims and objectives: The study aimed at assessing the knowledge of Primary school teachers regarding Selected Common Behavioural Problems of Children, evaluate the effectiveness of Structure Teaching Programme on Selected Common Behavioural Problems of Children and find out association between pre test knowledge score and selected demographic variables. Material and Methods: A one group pre-test post-test pre-experimental design and evaluative approach was adopted. The study was conducted among 60 primary school teachers conveniently selected from five primary schools of Vadodara. Results: The results of the study shows that in pre test, primary school teachers were having on average 49.40% knowledge regarding selected common behavioral problems of children and mean score was 14.82±3.372 and in post test, average 75.83% knowledge regarding regarding selected common behavioral problems of children and mean score was 22.75±2.802. T calculated value is 33.233 which is more than the tabulated value of 2.00 at 0.05 level of significance. Conclusion: This study concluded that structure teaching program is effective tools to improve the knowledge of primary school teachers regarding Selected Common Behavioural Problems of Children.Keywords

Mr. Rajesh Bhai Bhariya a 20year old man, whose occupation was building constructor labor, came to hospital on 19th September 2016 and his IPD No: 1610190125, diagnosis as Tetanus he belongs to Dahod district, Gujarat. Came to the hospital with C/O Body ache, Rigidity of generalized body since 5 days and difficulty in opening of mouth since one week. In nursing assessment found that: Tachycardia, Altered conscious, Disoriented, Hypertonic and Febrile. Nursing care provided as per need of the patient such as maintained personal hygiene, Provided regular oral care, Provided back care once in every four day, Sponge bath whenever required, Nasogastric feeding, Maintain I/o chart, IV fluids , Psychologically supported to patient family and regular Tracheotomy care done. As per consultants prescription medications administered such as Antibiotic cefaperazone+salbactum 3gm administered. Later patient recovered well and prognosis was good.Keywords

Background: In the Asia for more than 4.2 billion populations is suffer with the ACS. Acute coronary syndrome (ACS) is now a major cause of death in the hospital the mortality is more than 5%.1 So it is important to enhance the knowledge and practice regarding ACS algorithm among staff nurses.
Method: Qualitative research approach with pre- experimental one group pre-test and post-test design with non-probability convenience sampling was used to collect the 70 samples. A structured knowledge questionnaires and Practice checklist was prepared to assess the knowledge and practice of staff nurses.
Result: With regards to the pre-test assessment the score of 24(34%) subject was having moderate level of knowledge and 46(66%) subjects were having inadequate knowledge while in post-test 5(7%) of subject having moderate level of knowledge and 65(93%) subject having adequate level of knowledge The obtained pre-test mean score was 7.14 and post- test mean score was 15.24 the mean difference of the pre-test and post- test is 8.10 which shows the improvement in the level of knowledge among subjects, The pre-test SD was 1.35 and post- test SD 1.33 The obtained pair “t” test value 33.270, df=69 significant at 0.05 level. Whereas the pre-test score of practice shows 23(32.85%) subjects had inadequate practice and 47(67.14%) subjects had moderate practice, the post test data revels that 24(34.28%) subjects were had moderate practice and 46(65.71%) subjects had adequate practice towards ACS. The obtained pair “t” test value 27.790, df=69 shows significant at 0.05 level. It indicates that there was increased in the level of knowledge and improves practice towards acute coronary syndrome after providing nursing care algorithm. So that H1 research hypothesis accepted. The pre-test practice score had not significant association all demographic variable so H2 research hypothesis was rejected.
Conclusion: The findings of the study concluded that majority of subjects had inadequate level of knowledge and adequate practice. The nursing care ACS algorithm was effective among staff nurses in improving knowledge (t (69) = 33.270) and practice (t (69) = 27.790) significant at 0.05 level regarding acute coronary syndrome.

Background: Chronic kidney disease affected 753 million people globally in 2016, including 417 million females and 336 million males.1 So it is important to enhance the knowledge and practice regarding nursing management of patient during hemodialysis among staff nurses.
Method: Quantitative research approach with pre-experimental one group pre-test and post-test design was adopted and sample was selected by using non-probability convenience sampling technique which consists of 90 samples. Aself-structured knowledge questionnaires and practice checklist was prepared to assess the knowledge and practice of staff nurses.
Result: With regards to the pre-test knowledge, the score of 19 (21.1%) staff nurses had adequate level of knowledge and 71(78.9%) had inadequate knowledge, while in post-test 80(88.9%) had excellent knowledge, 4(4.4%)had inadequate level of knowledge and 6(6.7%) of them had adequate knowledge. The obtained pre-test mean score was 8.61 and after providing SOP it increased to 13.83, the mean difference of the pre-test and post-test was 5.22. The standard deviation (SD) of pre-test & post-test was 35.99 & 18.8. The obtained paired “t” test value was 43.57, significant at 0.05 level. Whereas the pre-test score of practice shown 51 (56.67%) had inadequate practice and 39 (43.33%) had adequate practice, the post-test data reveals that 80 (88.9%) had excellent score towards practice. The obtained pre-test practice of mean score was 8.68 and post-test of practice mean score was 13.88 of staff nurses. The mean difference of the pre test and post test of practice score was 5.2 the pre-test SD of practice score was 0.89 and post-test SD 0.79 the obtained paired ‘t’ test value 45.11, shows significant at 0.05 level. It indicates that there was increased in the level of knowledge and practice towards nursing management of patient during hemodialys is after providing SOP.
Conclusion: The findings of the study concluded that majority of staff nurses were having inadequate level of knowledge and average practice. The SOP was effective among staff nurses in improving knowledge (t (89) = 43.57) and practice score (t (89) =45.11) significant at 0.05 level regarding nursing management of patient during hemodialysis.
