Fault tree analysis (FTA) method was developed in1962 at Bell Laboratories by H.A Watson of U.S. Airforce for the evaluation and estimation of system reliability and safety. In this paper, we present the general procedure for FTA, its application in various fields and the modifications that have been made through the time to overcome the inadequacies of the method. For this work, we consider the case-study of ball pen which we use for writing purpose & presented a simplified methodology to determine base cause failure of the system.

Abstract

Fault tree analysis (FTA) method was developed in1962 at Bell Laboratories by H.A Watson of U.S. Airforce for the evaluation and estimation of system reliability and safety. In this paper, we present the general procedure for FTA, its application in various fields and the modifications that have been made through the time to overcome the inadequacies of the method. For this work, we consider the case-study of ball pen which we use for writing purpose & presented a simplified methodology to determine base cause failure of the system.
